These tests are for screening. A result is not a diagnosis: only a psychiatrist or psychologist can make one, after talking with you.
Your answers are scored on this device and never sent. Only your result is saved, with the email address you confirm.
Confirm your email address
Before the test we send a 6-digit code to your email address. Your result is saved with it, so that our team can help you if you wish. The email with the code contains no health information.
Think about how your relative has been recently compared with how they used to be, and choose the option that fits them best for each question.
Choosing an answer takes you to the next question. You can go back at any time.
Your result
No change reported
You did not report any decline in your relative's memory or daily functioning. Minor forgetfulness is common with age. If you notice a clear change in the future, you can see a doctor together.
Questionable or very slight changes
Your answers point to slight changes in some areas. At this level it can be hard to tell normal ageing from early cognitive decline. If the changes continue or increase, an assessment by a neurologist or psychiatrist is advisable.
Very mild impairment
Your answers point to clear changes in more than one area, which may fit a very mild cognitive impairment. We recommend an assessment of your relative by a neurologist or psychiatrist. Some causes of forgetfulness, such as vitamin deficiency, thyroid problems, depression or medication side effects, can be treated.
Mild impairment
Your answers point to a mild level of impairment that affects daily life. We recommend a detailed assessment by a neurologist or psychiatrist; an early diagnosis matters for treatment and for planning care.
Moderate impairment
Your answers point to a moderate level of impairment; your relative may need support with many daily tasks. We recommend talking to a neurologist or psychiatrist and getting support with care.
Severe impairment
Your answers point to severe impairment and a need for a great deal of help with daily care. We recommend talking to a specialist about regular medical follow-up and a care plan; as a carer, make sure you get support too.

Take your result as a rough guide, not a diagnosis. How you feel over time, and a conversation with a specialist, tell much more than a questionnaire.
Talk it through with a specialist
If your result worries you, or you would like to talk it through, our psychiatrists can review it with you, free and without obligation.

About this test
This test is based on the six areas of the Clinical Dementia Rating (CDR): memory, orientation in time and place, judgement and problem solving, activities outside the home, home life and hobbies, and personal care. As someone who knows your relative well, you answer 6 questions in about 3 minutes. The result shows the overall level of change you report; it is not a diagnosis and does not replace an examination by a doctor.
